REGISTRATION AND PAYMENT Registration Form

All registration fees include a copy of the conference proceedings, the welcome reception (Boissonade Tower 26F in Nov. 29), the conference dinner (An elegant time cruising Tokyo Bay in Dec. 1) and all teas/coffees. The registration fee will be not paied back after October 15, 2004

- Early Registration Fee before September 1, 2004, JPY36000(Japan) or USD300(Overseas).
- Late or On Site Registration Fee, JPY42000(Japan) or USD350(Overseas).
- Student Registration Fee (Please show your student card at the Conference site), JPY24000(Japan) or USD200(Overseas).
- Registration Fee for Accompanying Family, JPY24000(Japan) or USD200(Overseas).

No registration will be confirmed until payment is received. All payments must be in Japanese Yen (Japan) or US Dollar (Overseas) using one of the following methods:
- CREDIT CARD. Just VISA and MASTERCARD are accepted.
- BANKERS DRAFT (Cashier's Cheque). These can be purchased from any bank. The cheque should be written in Japanese Yen or US Dollar, payable to INFORMATION Journal, Prof. Lei Li, Faculty of Engineering, Hosei University, Koganei, Tokyo 184-8584, Japan. E-mail: office@information-iii.org.
